Rising Up Support Pathways is an NDIS REGISTERED PROVIDER, who has been supporting vulnerable members of our community since our beginnings in 2022. Your Key Responsibilities:
* Work with a variety of our NDIS customers, being available consistently for Days / Sleepovers and Weekend Shifts.
* Be accurate and accountable, writing shift notes and goal reporting to ensure support notes are available for stakeholders, management and or for NDIS reporting.
* Understand and provide person-centred supports to vulnerable people in ILO / SIL and community settings and have clear understanding of NDIS Code of Conduct as a Worker and also seek to understand our Company's NDIS Conduct Requirements.
* Personal Care Support in and out of a home setting such as hands on or in a community setting respecting dignity and privacy of a persons individual needs.
* Assist with Meal Preparation and meal planning options for clients to support their dietitician plans or personal preference applying safe hygience practicesl.
* Assist Clients with Community Access Support such as shopping, social engagement, Medical Appointments acting with integrity and respecting choice, control and opportunity to fail so our clients can learnt.
* SIL, ILO and In-home Accomodation support for clients to learn the life skills they need to build their own confidence in a home.
* Understand Restrictive Practice, Behaviour Support and Risk Management.

* Follow workplace policies and procedures to ensure NDIS practice standards are achieved.
* Work well within a team environment or independently dependent upon working environment.
* Medication Support and Administration with clients - Achieve accountable and accurate recording of medication and documentation. Be accountable towards reporting changes in health needs and be proactive in following up on needs with Service Management.
* You will be committed towards continuous learning within the changing NDIS sector.
Essential Working Criteria:
* Hold or begin Cert 3 Individual Support within 6 months of employment
* Must have NDIS Orientation Module and Basic Support Modules Completed, or will attain prior to starting with our team.
* Have approved NDIS Screening and Blue Card.
* Must have Car Licence and Reliable Transport for your shifts
* Higher Level Certificates and or experience will be highly regarded
* Be available to work Sleepovers / Weekend Shifts and Days within the requirements of the role.
* Commitment towards supporting participants of varying complexity highly regarded.
* Must be able to utilise microsoft and also company shift and scheduling software to record accurate and timely shift notes.
