About the Role
We are seeking an experienced, relationship‐driven professional to support the growth and delivery of high‐quality disability and complex care services.
This senior, client‐facing role is suited to someone with strong disability sector experience, established industry networks, and a genuine commitment to supporting people with acquired injuries, rehabilitation needs, complex care requirements, and long‐term support needs.
The role focuses on developing partnerships, building referral pathways, and strengthening relationships with Specialist Support Coordinators, hospital discharge teams, rehabilitation clinics, allied health professionals, case managers, and other community‐based referral partners.
Additionally, you will support the internal team as a senior service coordination resource, ensuring clients, families, support coordinators, and Disability Support Workers receive responsive, professional, and person‐centred support.
Key Responsibilities
* Develop and maintain strong referral relationships with Specialist Support Coordinators, hospital discharge officers, rehabilitation clinics, allied health providers, case managers, community organisations, and other relevant sector partners.
* Identify and convert new partnership and referral opportunities aligned to disability, rehabilitation, acquired injury, and complex care services.
* Represent the organisation professionally in meetings, networking events, hospital and community settings, and stakeholder forums.
* Support clients and families through initial enquiries, service discussions, onboarding, and transition planning.
* Assist with complex care coordination, including participant matching, service planning, risk considerations, and communication with key stakeholders.
* Provide senior‐level support to Service Coordinators and Disability Support Workers, particularly where complex client needs, communication, or care planning is required.
* Work collaboratively with internal teams to ensure referrals are handled efficiently, professionally, and with care.
* Contribute to service growth while maintaining a strong focus on quality, compliance, dignity, and client outcomes.
* Build market awareness and provide insights on referral trends, service gaps, competitor activity, and partnership opportunities.
* Maintain accurate records of stakeholder engagement, referral activity, client enquiries, and business development outcomes.
About You
You are an experienced disability sector professional with strong networks, sound judgement, and a natural ability to build trust with clients, families, referrers, and clinical stakeholders.
You bring empathy, commercial awareness, and practical service delivery experience. You understand that growth in this sector must be built on quality, trust, responsiveness, and genuine care.
You will be confident in both relationship development and operational support, with the ability to move between business development, client engagement, service coordination, and team support.
* Minimum 5 years' experience in the disability sector, preferably across complex care, community care, acquired injury, rehabilitation, or supported independent living environments.
* Demonstrated understanding of the NDIS, participant supports, support coordination, complex care requirements, and community‐based disability services.
* Existing networks with Specialist Support Coordinators, hospitals, rehabilitation clinics, allied health providers, discharge planners, or community referral partners.
* Experience supporting clients with complex needs, acquired injuries, rehabilitation requirements, or hospital‐to‐home transitions.
* Ability to support and guide Service Coordinators and Disability Support Workers in a senior capacity.
* Excellent communication, relationship‐building, and problem‐solving skills.
* Strong client‐facing presence, with a caring, professional, and empathetic approach.
* Current NDIS Worker Screening Check, Working with Children Check, and relevant police/background checks, or willingness to obtain.
* Current driver's licence and ability to travel for stakeholder meetings, client visits, and partnership development.
Preferred Qualifications
* Disability, Community Services, Case Management, Health, Allied Health, Nursing, Rehabilitation, Social Work, Psychology, or related fields.
* Training or experience in complex care, acquired brain injury, spinal cord injury, positive behaviour support, psychosocial disability, or high‐intensity supports.
