About the Role
As a Compassionate Care Coordinator, you'll guide customers through their care journey with empathy and understanding. You'll work closely with families, clinical teams, and external providers to develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.
Key Responsibilities
* Build trusted relationships with customers and support them to achieve their goals at home.
* Conduct onboarding assessments, develop comprehensive health plans, and coordinate multidisciplinary services.
* Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
* Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
* Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
* Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
* Promote safety, quality, compliance, and a culture of continuous improvement across all activities.
