Support Worker - Homelessness Initiative
We are seeking a dedicated Support Worker to join our homelessness initiative in Darwin. As a key member of our team, you will work closely with young people who are facing homelessness, providing person-centred support and case management services.
* Deliver tailored case plans to address the unique needs of each client;
* Conduct thorough assessments to identify areas for improvement;
* Facilitate connections to essential services, such as housing, healthcare and education;
To succeed in this role, you will need:
