You’ll join a supportive team of health professionals in a friendly and relaxed rural environment, committed to improving local health outcomes.About The RoleAs the GP Practice Nurse, you’ll be instrumental in providing timely access to quality primary healthcare services. Working within a multidisciplinary model, you will deliver nurse-led and GP-directed activities including immunisation, wound care, chronic disease management, health assessments, health education, triage, clinical risk identification, and recall management. Your work will ensure continuity of care for patients of the GP practice, supporting their health and well-being through proactive and coordinated service.About YouYou are a skilled and compassionate nurse with strong primary health care experience. You work well both independently and collaboratively, managing competing priorities with professionalism and care.
