As our Clinical Care Partner, you'll play a vital role in leading high-quality, person-centred care for older Australians living independently at home.
You'll combine clinical expertise with compassionate leadership — guiding our care team, supporting clients to achieve their goals, and helping shape the future of home and community care through innovation, quality, and collaboration.
What you'll be doing
Clinical Oversight and Governance
* Lead the delivery of safe, high-quality, and evidence-based care across our Home & Community Care services.
* Ensure care plans and clinical practices reflect best practice and meet aged care quality standards.
Customer Advocacy and Person-Centred Care
* Champion person-centred care that empowers clients to live well and independently at home.
* Work closely with clients, families, and health professionals to deliver care that reflects each individual's goals and preferences.
Leadership and Workforce Development
* Inspire and mentor Care Partners and Support Workers to deliver outstanding care.
* Support staff development through coaching, clinical guidance, and competency building.
Collaboration and Consultation
* Partner with a multidisciplinary network of health professionals to ensure holistic, coordinated care.
* Contribute your clinical expertise to care planning, case reviews, and service improvement discussions.
Quality, Compliance, and Risk Management**
* Maintain accurate and timely clinical documentation and uphold high compliance standards.
What you need to be successful in this role
Essential Criteria
* Registered Nurse, or Allied Health Professional (e.g., Occupational Therapist, Physiotherapist) with current AHPRA registration (or equivalent).
* Significant experience in community aged care, including Home Care Packages and CHSP programs.
* Proven experience providing clinical oversight and care coordination within a multidisciplinary team.
Desirable Criteria
* Experience in clinical governance, quality improvement, or compliance monitoring.
* Postgraduate qualification or further training in aged care, community health, or leadership.
* Demonstrated ability to conduct comprehensive clinical assessments and develop individualised care plans.
* Strong leadership, mentoring, and interpersonal communication skills.
* Proficiency in electronic care systems (Sandwai) and Microsoft 365.
* Current National Police Check, driver's licence, and reliable vehicle.
* Up-to-date COVID-19 and Influenza vaccinations (highly recommended).
