Your recent role
Wintringham is seeking a motivated and enthusiastic person to join our multi-disciplinary community team as a Casual After Hours Case Manager. In this dynamic role, you will be responsible for ensuring all community clients are able to access telephone support after hours between 5pm and 9am Monday to Friday, over the weekend and from 5pm the last working day before any Public Holiday / s to 9am on the next working day. This essential position will require a candidate who can assess a situation remotely, address immediate issues, use the escalation process appropriately, and provide a debriefing after an incident.
Your duties and responsibilities will include :
- Ensuring all phone calls received on the afterhours phone are answered in a timely manner
- Managing afterhours sick leave roster for staff working in the community after hours.
- Utilising a risk based management approach when addressing each individual request and taking the appropriate action based on the severity of the request
- Documenting on the After-Hours Service Log Form and enter notes under the relevant client in Epicor.
- Having a thorough understanding of the escalation process. Supervising and provide leadership to community care staff working after hours.
We are looking for candidates with the following :
- Current Home Support – Case Manager Wintringham employee (mandatory)
- An approved tertiary qualification in Social Work, Allied Health or Nursing.
- Experience working in mental health(desirable)
- Demonstrated experience in risk assessment and case management.
- A non-judgmental approach with the ability to work with people from varying socio-economic backgrounds
- Skills in assessing clients’ psychosocial, care, support, health and housing needs.
