Total remuneration package of $123,036 per annum, including superannuation and optional salary packaging.
Responsibilities
Senior Care Partner supporting older people to live safely, independently, and confidently in their own homes, overseeing a caseload of participants receiving Support at Home services, completing assessments, care planning, reviews, and coordinating tailored services that put participant choice first.
Qualifications and Requirements
* Appropriate qualification compliant with Australian Qualifications Framework (e.g., Diploma of Case Management and/or Community Services / Nursing / or related field, 3 or 4 year undergraduate degree in a health related field).
* Experience in Support at Home, case management, aged care, community services, nursing, or related field.
* Strong relationship-building and problem-solving skills.
* A passion for client-centred care and helping people achieve their goals.
* Current Victorian Driver's Licence.
