We are seeking an experienced Clinical Care Manager to oversee government-funded Home Care Packages, combining clinical skills with care planning and coordination.
The role involves providing comprehensive client assessment, developing care plans, and delivering advanced holistic care skills within a Case Management framework.
You will work closely with clients, families, and healthcare professionals to ensure seamless care coordination. Your expertise in budget management and ability to navigate the legal framework will be invaluable in this role.

To succeed in this role, you will need a Bachelor's degree in nursing, current Registration with AHPRA, and up-to-date Flu vaccination. You will also require Australian work Rights, a Police Check, and own vehicle and Australian Drivers' licence.
