The Dementia Together program is designed to help support recently diagnosed dementia patients and their careers. Dementia Together is a wellness retreat and education based program that provides dual-care to anyone living with the early stages of dementia and their carer. About the Role
The Engagement Facilitator works closely with the Dementia Together team using specialised skills and knowledge to guide the creation of personalised engagement strategies for people living with dementia attending the program. This will be achieved by conducting assessments, providing expertise to staff and clients enhancing the overall quality of support offered to carers and people living with dementia.
* Empower and support people living with dementia through the facilitation of group sessions and individualised support, while proficiently managing program materials, resources and technological tools.
* Skilfully coordinate, conduct, record and interpret a variety of assessments such as the Zarit Burden Interview (ZBI), DEMQoL Quality of Life Measure, Lawton Brody Instrumental Activities of Daily Living Scale (IADL), Global Deterioration Scale (GDS) and risk assessments, demonstrating proficiency in both face-to-face and phone interactions.
* Networking collaboratively with service providers, stakeholders, and the team to gather relevant, community-specific information for use in the Dementia Together program to support people living with dementia and to grow program awareness.
* Pursue ongoing professional development to stay abreast of advancements in dementia care, engagement methodologies, and community resources, ensuring continuous improvement and excellence.
What We're Looking For
* Demonstrated experience in the assessment, design, delivery of personalised engagement strategies and plans for people living with dementia.
* Qualifications in an appropriate field including but not limited to allied health, nursing, education, leisure and lifestyle or educational qualifications to a diploma level or higher in adult education for accredited programs.
* Completion of Certificate IV in Leisure and Health or similar related discipline (Highly Desirable), a formal qualification in dementia would be highly regarded.
* Hold a certificate of attainment for nationally accredited unit of competency "CHCAGE011 – Provide Support to People Living with Dementia" (or relevant unit of competency superseding this credential), or the willingness to obtain.
* Completion of Certificate IV in Training and Assessment, or similar related discipline (Desirable)
* Current Driver's license (QLD)
In addition to the above, you will need to demonstrate:
* High level of facilitation skills to conduct flexible, adaptable discussions and engagement activities sensitively and empathetically. Create a safe and supportive environment tailored to individual cognitive abilities, interests, and needs, fostering meaningful participation among people living with dementia and their carers.
* High level of knowledge of dementia & dementia care, encompassing the dementia subtypes, fundamental understanding of dementia neuropathy and common symptoms.
* Demonstrated ability to coordinate, conduct and interpreting diverse assessments relevant to dementia care, demonstrating an understanding of various evaluation tools.
* High level of proficiency in collaborating and fostering relationships not only within the immediate Dementia Together team, but also with external stakeholders, service providers, and community organisations.
The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
