About the role
The Community Support Coordinator (CSC) will connect participants with CCPC services that meet their disability support needs and assist them with working towards their goals.
The role will be required to undertake a range of tasks including but not limited to the following:
Conduct initial visits and complete annual renewals (consent forms, develop person-centred plans, risk assessment, service agreements, NDIS Portal Bookings and PACE endorsements)
Develop individual support plans and budgets
Monitor Service Agreement funding amounts to prevent over or under utilisation of funds
Complete referrals to Rostering team for support.
Attend stakeholder meetings in relation to participant’s support needs
Consult with participants and identify their goals, strengths, and support networks
Monitor and report progress against NDIS goal plans and budgets for all supports provided by CCPC
Key Selection Criteria
Relevant tertiary qualification, e.g., Certificate IV in Mental Health, Community Services, disabilities, or equivalent
Minimum 2 years’ experience supporting NDIS participants
Extensive experience within a high-level administrative capacity
Demonstrated understanding of NDIS support items and the NDIS Guidelines
Knowledge of issues experienced by people with disabilities (inclusive of psycho-social disability), their families and carers.
To be obtained and maintained at your own expense:
First Aid Certificate
A valid Working with Children Check (WWCC)
Criminal Record Check (CRC) issued within the last 12 months
A valid NDIS Worker Check (NDISWC)
