About the Role
We are seeking an experienced Aged and Disability Clinical Lead to join our team. As a key member of our Specialised Support Services team, you will be responsible for providing comprehensive clinical assessments to determine client care needs and ensuring a person-centred approach.
Key Responsibilities:
* Provide expert advice and support to clients, empowering them to navigate the service system and make informed decisions aligned with their needs and preferences.
* Review and ensure compliance with clinical policies, procedures, and health documentation, including discharge summaries, medical reports, and clinical care feedback for A&DS.
* Support the Care Partner team through case conferencing, clinical care planning, and involvement in client care, including ad-hoc nursing assistance when required.
* Work collaboratively with clients to understand their individual needs, goals, and aspirations, ensuring they are actively involved in decision-making about their care.
* Advocate within the organisation and externally to uphold clients' rights, ensuring that services and systems prioritise client choice, dignity, and autonomy.
Requirements:
* Tertiary qualifications in Allied Health, Nursing (RN Division 1), or a related field.
* Extensive knowledge and experience in Consumer Directed Care for Aged and Disability clients.
* Experience in conducting clinical assessments and developing, monitoring, and reviewing care plans in alignment with client needs and service delivery guidelines.
