Central Gippsland Health (CGH) is a sub-regional health service providing acute and aged care services in the Wellington Shire.
The CGH Palliative Care Service operates 24/7, delivering high-quality client and carer-focused care predominantly in home-based settings.
As a Community Palliative Care Nurse, you will work predominantly in clients' homes, supporting clinical, emotional, and practical needs while working closely with the Clinical Nurse Consultant – Palliative Care and the wider multidisciplinary team.
* Provide effective day-to-day clinical and emotional support to palliative care clients and their caregivers.
* Establish and maintain therapeutic partnerships with individuals and their families, promoting person-centred care and decision-making.
* Deliver complex supportive care using advanced palliative care knowledge across the continuum of care, including bereavement support.
* Collaborate with multidisciplinary teams to develop and implement coordinated care plans.
* Strong communication skills
