They can effectively manage and coordinate the service delivery model of case managed mental health wellbeing, alcohol and other drugs and social needs of clients.DutiesProvide screening and assessments with client’s referred to social health in determining the support required.Deliver comprehensive assessments for clients identified for case management, determine a goal-oriented case management plan and deliver targeted supports to a wide range of clients and presentations, as per the case plan.Respond appropriately to critical incidents, providing wellbeing first aid to clients in distress.Manage high quality case management practices.Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems.Work in integration and assisted referrals and coordination between local medical clinics to facilitate the delivery of an efficient, responsive and integrated mental health and social needs service to patients.Liaise with other community organisations and government agencies for the purposes of information gathering, referral, networking and promoting ATSICHS Brisbane Services.Maintain self - Management and Provide Team Contribution.Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of staff.CultureEnjoy working in an organisation that makes a difference in the community. Join our friendly and supportive Social Health team that will help you grow personally and professionally.BenefitsBase salary of $82,964 per annum + additional 11.5% Superannuation.Access to Not-for-profit salary packaging (Increases your take home pay).Mobile Phone provided.Supportive and collaborative team of dedicated professionals.COVID-19 RequirementImmunisation history showing you have received two doses of a Covid -19 vaccine is encouraged.Skills and Experiences:Required:Bachelor qualification in Early Childhood or equivalent either in Social Work/Human Services/ Behavioural Science/ Social Science/ Psychology/ Criminology/ Allied Health/ AOD/ Occupational Therapy/ Sociology/ Public Health or relevant field.Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.Ability to work with diverse people and presentations in providing case managed social work driven supports.Demonstrated ability to record accurate and consistent case notes.Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.A willingness to travel across the ATSICHS Brisbane region of services and work across different sites.A willingness to provide social work supports and engagement to patients outside of a clinic environment.Working with Children’s Card (Blue Card) or be eligible to apply.NDIS Working Screening Card (Yellow Card) or be eligible to apply.National Police Clearance or be eligible to applyDriver's Licence.Preferred:Previous experience in a multi-disciplinary team environment.Previous experience working in an Aboriginal Medical Service.How To ApplyIf this sounds like you, please submit your application by following the links on the recruitment page.
