The Role
We are looking for an experienced Nurse (Enrolled or Registered Nurse) to join our Royal Park Medical Centre to be the driving force and advocate for the local community in preventative health. This role involves supporting GPs and their patients to identify and manage chronic conditions, aiming to keep them out of hospital. The position is preferably full-time but part-time options can be discussed for the right candidate.
The Tasks
* Prepare Care Plans and Team Care Arrangements to help patients manage their diseases
* Empower patients to take ownership of their health
* Coach patients on health behavior change principles
* Enable patients to access ongoing care from a multi-disciplinary team
* Assist in early identification of chronic disease through health assessments
* Work with GPs to promote preventative healthcare
* Increase the number of patients receiving a care plan for the first time
* Review patient care plans regularly
* Manage administrative tasks related to CDM appointments

The Successful Candidate
* AHPRA registered Enrolled or Registered Nurse
* Experience in Chronic Disease Management and health assessments
* Knowledge of Health Assessment is highly regarded
* Strong organizational and time management skills
* Excellent communication and customer service skills
* A passion for people and a personable approach
