The role of this position involves monitoring and care coordination of consumers with severe and enduring mental illness.
Monitoring and care coordination are conducted in a consumer's daily environment, fostering a community approach to their recovery.
This position requires a multidisciplinary, recovery orientated and holistic approach to the care of its consumers.

AHPRA registration as a Registered Nurse, Psychologist, Occupational Therapist or eligibility for membership with AASW is required for this role.
