War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services include 24‐hour inpatient care, Monday‐to‐Friday outpatient departments and several specialist community teams, such as Transitional Aged Care, an Aged Care Assessment Team, a Geriatric Flying Squad, Young Onset Dementia, a 7‐day‐a‐week day centre, 24‐hour accommodation for regional guests and full range of hospital support services. It is set within beautiful tranquil grounds and operates in a well‐supported team environment.

About the Role

The Rehabilitation Clinical Nurse Consultant Grade 2 (CNC2) is a senior clinical leadership position at WMH. The CNC2 provides advanced consultancy, leadership and expert nursing practice across both inpatient aged rehabilitation (Morgan Unit) and the Transitional Aged Care Program (TACP). The role leads clinical excellence through the entire patient journey, from inpatient rehabilitation to community‐based transitional care, supporting complex care coordination, optimal functional outcomes and person‐centred service delivery.

Responsibilities
* Lead clinical excellence across inpatient rehabilitation and community‐based transitional care.
* Collaborate closely with the Nurse Unit Manager, TACP Manager and WMH Executive to drive clinical service development and workforce capability building.
* Provide high‐level clinical guidance to multidisciplinary teams and lead quality improvement initiatives.
* Contribute to strategic planning, research and education aligned with ARNA competencies and NSW Health frameworks.
* Lead clinical supervision, education and service innovation, fostering a culture of excellence, accountability and continuous improvement.
* Represent WMH at local and district forums and support the development of integrated models of care across inpatient and community settings.
Qualifications
* Current registration with AHPRA as a Registered Nurse with a minimum of 5 years full‐time equivalent post‐registration experience, including at least 3 years in aged rehabilitation, gerontology, community care or a related specialty.
* Approved postgraduate nursing qualifications relevant to Aged Care, Rehabilitation or Chronic/Complex Care, or commitment to commence within 12 months.
* Current Class C Driver's Licence (required for community‐based TACP activities).
Selection Criteria
* Demonstrated advanced clinical leadership, expert clinical reasoning and the ability to deliver, evaluate and guide high‐quality patient care across inpatient and community settings.
* Demonstrated commitment to professional development, capability building and clinical supervision for self and others.
* Proven experience in quality improvement, service development, research translation and the ability to lead multidisciplinary improvement initiatives.
* Demonstrated ability to work autonomously and collaboratively within complex multidisciplinary and cross‐site environments.
* Excellent interpersonal, written and verbal communication skills, including high‐level computer literacy.
* Demonstrated commitment to consumer participation, person‐centred care, and engagement in service planning, evaluation and policy development.
Health and Safety Requirements

All Category A NSW Health workers are required to have received an annual seasonal Influenza vaccination or have an approved medical contraindication certificate. COVID‐19 vaccinations are strongly recommended for all NSW Health workers.
