Job Title: Community Mental Health Specialist
We are seeking a highly skilled and compassionate clinician to join our team as a Community Mental Health Specialist.
* Provide home-based mental health care services to older adults in the community, with a focus on preventing hospitalization and promoting patient-centered care.
* Work collaboratively with interdisciplinary teams, including nurses, social workers, occupational therapists, and psychiatrists, to deliver high-quality care.
* Develop and implement individualized treatment plans that address the unique needs of each patient.
Requirements:
* Degree in Social Work or related field.
* Current registration with AHPRA.
* Experience working in acute mental health settings, preferably with older adults.
* Strong communication and interpersonal skills.
