We are seeking a compassionate and dedicated Support Worker to join our team, providing support to individuals with intense and complex needs. The successful candidate will work directly with individuals who experience a range of challenges, including physical and mental health conditions, behavioral difficulties, and learning disabilities, ensuring they are supported in a safe, respectful, and empowering environment. Key Responsibilities:
Provide Direct Support:
* Help participants with daily activities, such as personal care, getting around, preparing meals, and cleaning.
* Assist participants in reaching their NDIS personal goals, like developing new skills, becoming more independent, and taking part in community activities.
* Offer companionship and emotional support, encouraging positive relationships and social interaction.
* Guide and monitor participants in activities that improve their physical and mental health, such as hobbies, exercise, and learning.
Care Plans and Risk Management:
* Work with other professionals (like social workers and Allied health professionals) to create and follow care plans based on the participant's needs and wishes.
* Track and report on the participant's progress, noting any changes in their health or behavior.
* Identify risks to their health and safety and take action quickly in emergencies.
Behavioral and Emotional Support:
* Help manage difficult behaviors in a calm and understanding way, using techniques that focus on positive outcomes.
* Use strategies that encourage positive behavior and support emotional well-being.
* Stay calm, patient, and supportive during stressful or crisis situations.
Health and Medication Support:
* Assist participants with taking prescribed medications and watch for any reactions.
* Ensure that health appointments and services are kept up to date.
* Report any changes in the participant's physical or mental health to the right medical staff.
Collaboration and Teamwork:
* Work closely with a team of professionals, attending meetings and helping review care plans to make sure the participant's preferences are heard.
* Keep in contact with families and other important people in the participant's life to ensure a well-rounded approach to care.
* Help other team members by sharing helpful ideas and tips on supporting individuals with complex needs.
Documentation and Reporting:
* Keep accurate records of all support activities, progress, and any incidents.
* Follow guidelines for reporting accidents, incidents, or changes in health.
Promote Independence and Empowerment:
* Encourage participants to make choices about their lives, helping them feel more confident and independent.
* Support participants in learning new skills and taking on more responsibility in their daily lives.

Pay: $25.00 – $36.00 per hour
