We are currently seeking a highly skilled Clinical Pharmacist to join our Clinical Services team. As a key member of the team, you will be responsible for providing clinical pharmacy services across various wards and departments, including inpatient and outpatient areas.

Your Key Responsibilities

Key Responsibilities:

* Compile and document best possible medication history for hospital inpatients
">
* Reconcile and clinically review prescribed medications
">
* Manage supply of inpatient and discharge medications for hospital patients and some outpatients
">
* Check prepared medications prior to delivery to wards and patients
">
* Prepare discharge medication and summaries
">
* Provide medication education and counseling for patients and carers
">
* Liaise with external facilities and community healthcare providers to ensure continuity of care for individual patients
">
* Provide medication-related information, advice, and support to the multidisciplinary team
">

About You
">
To be considered for this role, you must have a Bachelor of Pharmacy and APHRA registration to practice as a Pharmacist (without restrictions). Highly regarded attributes include prior demonstrated experience in ward-based clinical pharmacy, flexibility to work all shifts, including evenings, weekends, public holidays, and on-call (with training), and the ability to work at all Mater Hospital locations.
