Job Title
We are seeking a dedicated and compassionate professional to lead our team of support workers in delivering high-quality care services to clients.
The successful candidate will be responsible for planning, coordinating, and implementing care services in the home and community setting.
* Organising, conducting, and documenting client assessments;
* Ensuring compliance with relevant legislation, policies, and procedures;
* Providing advocacy and support for clients and their families;
* Coordinating stakeholders and external services to enhance care delivery;
* Promoting the service and identifying opportunities for expansion;

Requirements
* A Certificate III in Individual Support (Home and Community) or equivalent qualification;
* A current driver's licence and willingness to use own vehicle for work purposes;
* Applied knowledge of aged care legislation, funding mechanisms, government policies, and statutory regulations;
* Current First Aid and Cardiopulmonary Resuscitation (CPR) certificates;
* Minimum one-year experience in leading a service delivery team and planning, co-ordinating, and implementing care services in the home care/community care environment;
* Ability to use mobile devices to record and manage information;
* Intermediate computer skills with ability to use Word, Excel, Outlook, and PowerPoint;
* A current National Police Certificate - or ability to acquire;
* Ability to undergo a National Personal Insolvency Check.
