Job Overview
We are looking for a passionate, highly motivated Band 6 Registered Mental Health Nurse to join our Dementia Assessment Service based in North West Sussex. Applications are invited for individuals who have a keen interest in dementia and possess post-registration experience working with clients with dementia or within dementia services. The Dementia Assessment Service (North) provides diagnostic assessment and post-diagnostic interventions for people with suspected dementia within the North Sussex area in partnership with the Alzheimer's Society. Early diagnosis is a fundamental component of the National Dementia Strategy and aims to empower people and their carers to make decisions about their future based on early diagnostic information relating to their condition. With this in mind, our service aims to provide support to service users and carers with the early detection of dementia and the provision of post-diagnosis follow‑up. Our work is focused on assessment of potential dementia; although we support a large number of people, we do not carry caseloads for long periods of time. We use a standardised assessment process in conjunction with a multi‑disciplinary team to help resolve concerns around cognition and potential dementia. Once a diagnosis is formulated, we provide appropriate support, signposting and advice to patients and their families to help them live well with the diagnosis.
Responsibilities
* Conduct comprehensive clinical assessments of patients suspected of dementia.
* Coordinate assessment processes with multidisciplinary teams.
* Prepare and interpret assessment documentation.
* Provide advice, support and signposting to patients and families following assessment.
* Maintain accurate and timely clinical records.
* Support community visits and deliver care in line with NHS guidelines.
* Participate in continuous professional development and specialist dementia training.
Qualifications & Experience
* Registered Nurse with current registration (NMC – Mental Health or LD).
* Completion of a mentorship course/ENB equivalent.
* Significant post‑qualifying experience in mental health or learning disability care.
* Experience conducting clinical assessments, including risk assessments.
* Experience working consultatively with professionals and within statutory and non‑statutory teams.
* Experience with the CPA process and clinical governance.
* Ability to travel across the region and, where applicable, work remotely in agreement with line manager.
Desirable Experience
* Experience working with older adults presenting with dementia or complex mental health difficulties.
* Experience in the community setting.
* Knowledge of partnership working within and across various agencies.
