Job Description:
We are seeking a skilled Clinical Pharmacist to join our multidisciplinary healthcare team in the Southport and Formby area.
The postholder will work as part of a team, advising and supporting colleagues to promote high-quality, safe, and cost-effective care for patients. They will become part of a group of PCN Clinical Pharmacists and Technicians working under the supervision of the Clinical Pharmacy Team Manager.
The ideal candidate will have experience working with GPs and patients to address medicine adherence and possess an appreciation of primary care prescribing concepts and strategies for improving prescribing.
Main Responsibilities:
* Work as part of a multi-disciplinary team in a patient-facing role
* Review medication and clinical processes for high-risk patients to ensure high-quality, evidence-based prescribing
* Conduct structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy
* Provide clinical medicines expertise to both patients and general practice teams
* Interface with community and hospital colleagues to resolve medication-related issues
Required Skills and Qualifications:
* Mandatory registration with the General Pharmaceutical Council
* Master's Degree In Pharmacy (MPharm) or equivalent
* Independent prescriber or working towards gaining independent prescribing qualification
* Membership of the Royal Pharmaceutical Society
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
