In this vital role, you will support the smooth transition of patients from hospital along their aged care pathway.
Your work will directly contribute to improved patient flow and care continuity across South Australian Local Health Networks.You will play a key role in discharge planning by coordinating admission paperwork, liaising with aged care providers, and ensuring appropriate community services are arranged.
With a focus on patient advocacy, you will work closely with families and carers to align care plans with individual preferences, all while delivering essential administrative and logistical support.
