Who are we?
Established in 1991, Danila Dilba HealthService (DDHS) is an Aboriginal community-controlled organisation committed toproviding culturally appropriate, comprehensive primary health care servicesand community development within the Biluru (Aboriginal) community of the YilliRreung (Greater Darwin) Region in the Northern Territory.
We work to address the social determinants ofhealth including the broader issues that affect our people's health, throughevidence-based research and public advocacy. We also work to empowerindividuals, families, and communities to optimise their health.
About the role
The SEWB SupportWorker will work in partnership with a multidisciplinary team to support Adultsto improve their social and emotional well-being.
Responsibilities:
* Maintain regularcontact with clients assessed as low needs and within the scope of practice whohave been referred by clinics, other providers, or self-referral to SEWB.
* Work with astrengths-based approach to develop, implement, and monitor a case managementplan to address specific client needs in collaboration with the SEWBmulti-disciplinary team.
* Provide interventionand support to clients who are at risk of psychosocial distress or experiencinglow acuity concerns. Within the worker's expertise and skills, identifyimmediate risks and needs, explore possible interventions, develop, andimplement support plans, and facilitate referrals.
* Provide SEWB supportthat is sensitive and responsive to the cultural needs of Aboriginal and TorresStrait Islander clients.
* Work closely withelders, community leaders, and other stakeholders to ensure services areculturally respectful and aligned with community values and traditions.
To besuccessful you will need:
* Identify asAboriginal
* Demonstratedexperience working with Indigenous people and the ability to communicateeffectively and in a culturally appropriately manner.
* Demonstratedknowledge of current issues, standards, and trends in the delivery of mentalhealth and social and emotional wellbeing services to Aboriginal people.
* Demonstratedawareness of and sensitivity to Aboriginal culture and history and knowledge ofissues impacting on Aboriginal children and families, youth, and adults andthose affected by Stolen Generations' policies and practices.
* Demonstrated abilityto work within and maintain strict confidentiality guidelines and policies.
* Demonstratedcommunication and interpersonal skills with the ability to liaise with peoplefrom a wide range of cultural and social backgrounds to both obtain and conveyinformation relevant to their care.
* Demonstrated abilityto interact effectively with clients, carers, and other professionals fromdiverse cultures.
* Ability to workindependently, and as part of a team, with an ability to plan, manage andprioritise tasks and resources to deliver outcomes within time and budgetconsiderations.
* Proficiency in andcommitment to the use of electronic information systems for the maintenance ofclinical and service delivery records.
* Experience workingin a community services program or community education program.
* Certificatequalifications in Community Services or equivalent.
What we offer:
* 6 weeks annualleave per annum
* 5 days paid leaveduring our end of year shut down period (in addition to annual leave).
* Salary packagingof up to $15,990 per annum.
Employment with DDHS is subject to a policecheck, a current NT working with children's card, CPR certificate, and fullvaccination against COVID-19, or an exemption as listed on the AustralianImmunisation Register (AIR).
