Position Overview
We are looking for dedicated Home Community Support Workers to provide high-quality, person-centered support to NDIS Participants. Your role will be crucial in helping them achieve their goals, enhance their quality of life, and improve their physical and mental health.
Key Responsibilities
* Individualised Support: Providing a range of services tailored to meet the unique needs of NDIS Participants, such as housekeeping, grooming, toileting, meal preparation, medication management, and more.
* Knowledge and Care: Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding restrictive practices and medication requirements.
* Community Inclusion: Assisting Participants in participating in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.
* Communication: Providing regular feedback to the Home and Community Support Manager regarding issues that require follow-up. This includes matters related to Participants, their families, external supports, fellow employees, and community access activities.
* Compliance: Adhering to UFC's Code of Conduct, policies, and Quality Management System procedures to ensure the highest standards of care and support.
* Safety and Wellbeing: Demonstrating a strong commitment to the safety and well-being of yourself, Participants, and others in the work environment.
About You
* You will thrive and succeed in our team by bringing the following experience to this position:
o Certificate III or IV in individual supports (aged care, disability or home and community services)
o Drivers License and vehicle
o Must be Female (Female client requesting a Female Support Worker)
o Experience working within the In-Home Care environment
o Ability to meet deadlines, work with minimal supervision and pressure.
o Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
o Understanding of WHS procedures
Desirable
* Qualification in community services, disability, mental health or related field
* Qualifications in nursing, assistant nursing or registered nurse
* Manual Handling
* WWC
* Experience working with CALD clients
* Complex care
* Dementia care
* Mental Health first aid
* Certificate IV in mental health
* Trauma informed care
* Mental health awareness and sensitivity training
