The role:
This isn't just a support role, it's about leading with care. You'll provide hands-on support to people of all ages and abilities, while guiding your team to deliver safe, person-centred care. Think of yourself as the team captain: mentoring others, keeping plans on track, and making sure everything runs smoothly.
This is a part-time, permanent position with flexibility to increase hours over time. Around 80% of your work will be direct support, with the remainder focused on admin and team leadership.
We're looking for six Team Facilitators across the Wheatbelt region to partner with their Service Coordinator and help grow and strengthen local services. Positions are based in:
Ellenbrook/Bullsbrook, Lancelin, Narrogin, York, Northam, Toodyay, Chittering, Wongan Hills, and Goomalling.
What you'll do:
* Support people with daily living and community activities
* Work with families and health professionals
* Keep plans and records accurate
What you need:
* Cert III in Disability, Aged Care, Mental Health (or working towards)
* Availability to work Monday to Sunday 8am-5pm
* NDIS Worker Clearance
* Working with children check
* Valid Driver Licence with a reliable vehicle
