Job Description:

This is a challenging post that requires collaboration with multi-disciplinary teams/agencies in addressing care/support issues, enabling individuals to remain in their own home through the promotion of independence; preventing unnecessary hospital admissions and improving the health and wellbeing of service users.

The successful candidate will be responsible for supporting the delivery of services in line with the organisation's objectives, working closely with colleagues to achieve common goals. They will also be required to undertake continuous professional development to maintain their skills and knowledge.

Required Skills and Qualifications:

- A Professional Social Work qualification (DipSW, CQSW, CSS, BA, MA or Foreign qualification certified by the Professional Social Work regulatory body)

- Current registration with the Professional Social Work regulatory body at all times

- An ongoing commitment to personal and professional development

- Good keyboard/IT skills including word processing, spreadsheets, email and PowerPoint

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker)) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration.

Person Specification:
