This is a maternity relief position. If the permanent incumbent returns early, the contract will cease with two weeks notice. Maternity relief positions can be for up to 12 months, with potential continuation for up to a further 12 months for maternity leave reduced hours; there are no permanency claims.
About The Role
As an OPMH Care Coordinator, you will work with people over 65 living with moderate to severe mental illness, including those in residential aged care. You will provide care coordination, case management, recovery-focused interventions, and support for families and carers. You will conduct mental health assessments, deliver evidence-based treatments, and make referrals in line with best practice standards. You will work both independently and within a multidisciplinary team to help older adults and their families navigate complex mental health needs and improve quality of life.

Qualifications and Experience
- Hold a relevant tertiary qualification (Psychology, Clinical Psychology, Occupational Therapy, Nursing, Social Work, Counselling, or Community Welfare) with the required registration or membership and at least three years post-graduate experience in mental health, or a Master’s in Clinical Psychology with endorsement.
- Experience in care coordination for older persons with complex needs.
- Understanding of the mental health needs of older adults and their families with person-centred care.
- Ability to work independently and as part of a multidisciplinary team.
