About the Role

This role involves providing holistic guidance, support and advice to people living with a disability and their families. The primary focus is on assisting them in navigating the National Disability Insurance Scheme (NDIS) as well as connecting them with other available community supports.



Main Responsibilities



1. Provide guidance, support and advice to customers to help them navigate the NDIS Access Pathway and Participant plans.

2. Support customers in completing and submitting documents required for Scheme Access Requests.

3. Refer individuals to Local Area Coordinators or Early Childhood Teams once necessary documentation is provided.

4. Maintain asset maps to ensure community connections are established for individuals aged 9 years and over.

5. Budgeting Community Connections Plans as needed.

6. Collaborate across multi-disciplinary teams ensuring a smooth management of the entire Early Childhood pathway for children under 9 years old.
