Belong at Home (CHC) is seeking a passionate and qualified Client Admissions Manager to join our dedicated team. This is a full-time opportunity for a RN/Allied Health Professional who is enthusiastic about making a meaningful difference in the lives of older Australians. This is a full-time position with a highly respected not-for-profit community organisation providing care and support services across the South West of WA. We operate throughout the South West with branches in Bunbury, Busselton, Margaret River, Collie, Donnybrook and Bridgetown.

We have an exciting opportunity for an enthusiastic and organised RN/Allied Health professional with knowledge or experience in the Aged Care sector, looking to make a real difference through their work at Belong at Home in Bunbury. This is a fulltime role, working as part of our client services team. The full-time role is based in Bunbury with travel required throughout the surrounding area to meet business requirements. Applicants must reside in the Bunbury area to be considered.
Key Responsibilities
- Act as a brand ambassador for Belong at Home by actively promoting the organisation and its services to clients and representatives.
- Work collaboratively with team members on compliance ensuring the highest standards are met.
- Process client referrals and service requests including point of contact for referral inquiries within specified timeframes to facilitate a comprehensive handover to the Service Coordinators/Care Partner.
- Assess and collaborate with clients, their representatives and community supports to plan and achieve the best outcomes possible for clients so they can remain independent and connected in their community.
- Use your clinical expertise to complete admission/onboarding assessments including clinical risk, client-centred personalised care plans relevant to budgets with accuracy and detail.
- Manage Client information on the My Aged Care Portal (MAC).
About you
- Registered Nurse/Allied Health Professional, CRM experience and strong administration skills.
- Meticulous in following procedures and documentation.
- Knowledge of Home Care Packages/support at home and Commonwealth Home Support Program.
- Customer service focussed, organised and well presented.
- Excellent verbal and written communication skills.
- A genuine enthusiasm, care and compassion in your commitment to working with the elderly.
- A current “C” class WA Driver’s License is essential with a current national police clearance and willingness to obtain annual flu vaccination.
