About the Role:
* We are seeking a highly skilled Registered Nurse to join our team as a Clinical Nurse Specialist in Home and Community Care.
About You:
* You will be working on a permanent full-time basis, enjoying consistent work in the community and visiting clients in their own homes under aged care packages.
* You will be responsible for conducting client care assessments, supervision and management of support workers, coordination of quality person-centred care according to client needs, and clinical in-home services including care plan assessment, medication, incontinence, chronic disease, complex wound management, palliative care, and diabetes.
* You will also develop, implement and evaluate plans of care alongside clients and their families, liaise with healthcare professionals to deliver quality care to clients, and ensure full compliance with organisation and care standards.
Requirements:
* Current AHPRA registration
* Experience working in Home and Community Care or Aged Care
* Strong clinical assessment skills
* Excellent engagement and communication skills
* Amazing customer service, friendly approach, compassion and values-focused
* Current WA driver's licence
* National Police Clearance within the last 6 months
* Current CPR and First Aid Certificate or willing to obtain
* Working rights in Australia
