Job Title: Community Support Worker, Muswellbrook
About the Role: We are seeking a dedicated and compassionate individual to provide support to clients with daily tasks such as personal care, community access, respite care, domestic assistance, medication assistance, meal preparation, transport, shopping assistance, welfare checks, and documentation of client progress/reports.
Main Responsibilities:
1. Assist clients with daily living activities, including personal care, grooming, and hygiene.
2. Provide transportation services for clients to attend appointments, social events, and other engagements.
3. Support clients with meal preparation, grocery shopping, and household chores.
4. Assist clients with medication management, including administration and reminders.
5. Document client progress and maintain accurate records.
6. Collaborate with healthcare professionals to ensure seamless care delivery.
Required Skills and Qualifications:
* Diploma or Certificate in Community Services, Aged Care, or related field.
* Minimum 1 year of experience in a similar role.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Basic computer skills, including proficiency in Microsoft Office.
