Role Summary
We are seeking an experienced Support Practitioner to join our team. In this role, you will be responsible for providing support and guidance to families and children in need.
Key Responsibilities
* Facilitate the intake and placement of children and young people into foster and kinship care families.
* Deliver assessments, crisis support, and therapeutic interventions as required.
* Develop and evaluate care plans tailored to individual needs and circumstances.
* Promote positive outcomes through evidence-based parenting programs and collaboration with community partners.
* Recruit, assess, and support foster and kinship carers, ensuring a smooth approval process.
* Lead case management planning and maintain accurate client records.
* Provide flexible, in-home support services, including after-hours and on-call duties.
* Culturally safe and inclusive practices are essential when working with Aboriginal and Torres Strait Islander communities.
Requirements
* A relevant qualification and/or experience in social work or a related field.
* Ability to work effectively with diverse communities and individuals.
* Strong communication and interpersonal skills.
* Maintaining confidentiality and handling sensitive information is crucial.
* Familiarity with relevant legislation and policies.
