Transitional Aged Care Assistant
The Transitional Aged Care Program is a multi-discipline, home-based therapy and nursing program for older people discharged from hospital.
This role is in the community setting, based on the Prince of Wales Hospital Campus, Randwick.
You will visit clients who have been discharged from hospital and are in the Transitional Aged Care Program for up to 12 weeks.
The program provides time-limited, multidisciplinary support and low-level therapy to clients living in the community.
* Engage directly in providing care, assisting clients with personal and domestic activities of daily living, function and mobility tasks, community access and client transport, supervised by the Registered Nurse and the multi-disciplinary team.
* Work within a multidisciplinary team to deliver therapeutic intervention and support to aged clients in their homes following sub-acute hospital care, aiming to improve physical function and independence.
* Support the nursing team in delivering nursing care in the community as directed by the Registered Nurse.
* Provide direct care activities to patients in accordance with the nursing care plan and under RN supervision.
* Assist Registered and Enrolled Nurses with patient care interventions as directed.
* Contribute to collecting accurate health information and maintaining medical documentation as required.
* Communicate effectively with patients and healthcare team members in accordance with appropriate protocol within the Transitional Aged Care Program scope.
Required Qualifications & Experience
* Certificate III in Health Services Assistance (Descriptor: Assisting in Nursing Work in Acute Care) or equivalent.
* Current unrestricted NSW Driver's licence, Class C.
* Current vaccinations as per NSW Health policy.
* A valid Driver's Licence and ability to travel within the Randwick - Botany LGA; use of hospital fleet vehicles may be required.

Location & Schedule
The role is based on the Prince of Wales Hospital Campus, Randwick, in the Community setting. Work days are Monday – Friday. You will travel within the Randwick - Botany LGA using hospital fleet vehicles. A current NSW Driver's Licence and vaccinations per NSW Health policy are required.
