You will work alongside a large allied health team.
This community caseload will cover: Transition Care Program, Rehab in the Home and Geriatric Evaluation and Monitoring at home.
Focus on over 65+, home visits, home modifications, ordering equipment, restorative care and supporting patients post discharge from hospital.
Ability to work independently in Community Programs Experience in Rehab/Restorative care/Outpatients - TCP/Rehab in the home/Aged care desirable FREE accommodation available – my current locums are in individual serviced apartments One way domestic travel reimbursed Locum rates $$ I've had great feedback from current locums.
