Job Title: Holistic Care Coordinator
We are seeking an experienced and passionate Holistic Care Coordinator to join our multidisciplinary team. As a key member of our community health programs, you will deliver high-quality, culturally safe care to the local Aboriginal and Torres Strait Islander people.
Key Responsibilities:
* Lead delivery of community health programs for chronic disease management, preventative health, and health promotion.
* Collaborate with GPs, Aboriginal Health Practitioners, Allied Health Professionals, and external partners to enhance access to care and drive better health outcomes.
* Provide clinical leadership, mentoring, and support to nursing staff and Aboriginal Health Practitioners.
* Participate in case conferencing, quality improvement activities, and care planning meetings.
* Educate patients through self-management guidance.
* Maintain accurate clinical documentation using Medical Director.
* Ensure compliance with AHPRA, clinical guidelines, and organisational policies.

Requirements:
* Current AHPRA Registration.
* Bachelor of Nursing (BN, BSc Nursing).
* Understanding of Aboriginal health, cancer pathways, and community needs.
* Strong communication, teamwork, and relationship-building skills.
* Australian citizenship or Permanent Residency.
