Case Manager Role Summary:
This is a challenging yet rewarding opportunity to make a positive impact on the lives of children and young people in Out-of-Home Care.
The successful candidate will be responsible for developing and implementing individualized case plans, working closely with families, carers, and agencies to achieve therapeutic and cultural goals.
Key Responsibilities:
* Develop and review case plans to ensure they are culturally sensitive and meet the needs of young people
* Provide coaching and support to staff to embed trauma-informed practices into daily care
* Engage with families and carers to build strong relationships and achieve mutually beneficial outcomes
* Maintain accurate documentation and ensure compliance with relevant standards and legislation
* Promote safe and nurturing environments that respect the diversity of all individuals
Requirements:
* Diploma or degree in a related field such as community, youth, or social work
* Proven experience in case management and stakeholder engagement
* Knowledge of trauma-informed care practices and their application in residential settings
* Strong written and verbal communication skills
* Current driver's licence and ability to obtain probity clearances
