Robinvale District Health Services' is seeking an experienced and innovative Nurse Practitioner to join our dynamic Community Health team. In this full-time role, you will be responsible for providing comprehensive primary healthcare services to the diverse population of Robinvale, Victoria.
Key Responsibilities
* Assess, diagnose, and manage a wide range of acute and chronic health conditions in the community setting
* Provide health education, promotion, and disease prevention services to individuals and groups
* Collaborate with other healthcare professionals to develop and implement holistic care plans
* Participate in the development and delivery of community-based health programs
* Contribute to the ongoing evaluation and improvement of community health services
* Maintain accurate and comprehensive patient records in accordance with professional and organisational standards
What We're Looking For
* Current registration as a Nurse Practitioner with the Nursing and Midwifery Board of Australia
* Significant experience in community or primary healthcare settings, with a strong understanding of population health principles
* Excellent clinical assessment, diagnostic, and management skills
* Proven ability to work collaboratively within a multidisciplinary team
* Strong communication and patient education skills
* Excellent communication and interpersonal skills to effectively collaborate with patients, families, and the multidisciplinary team
* Demonstrated ability to work independently and make autonomous decisions
* Commitment to evidence-based practice and continuous professional development
