* Full Time or Part Time Positions starting from $97,000 + Super (Negotiable based on experience)

Healthii People are proud to have recently partnered with Open Minds, one Australia's longest standing Not-For-Profits supporting communities for over 110 years. They are a leading provider of Mental Health and Disability Services across QLD and NSW.

In collaboration with other Mental Health Services, Open Minds are leading the development and oversight of the Clinical Care Coordination Program across the Country to Coast (CCQ) Primary Health Network (PHN). These services are designed to provide a comprehensive, integrated and holistic approach to mental health care for adults with moderate - complex mental health conditions.

We are pleased to be working alongside the Open Minds team to support the recruitment of Clinical Care Coordinators (Mental Health Clinicians) in the following areas across Central QLD.

* Sunshine Coast
* Hervey Bay
* Bundaberg
* Gympie
* Rockhampton
* Gladstone
* Emerald

As a Clinical Care Coordinator with Open Minds, you will work within a multi-disciplinary team that is not KPI centred but more focussed on providing high quality mental health care to the participants. In collaboration with the multi-disciplinary team, you will conduct intake, assessments and provide short to mid term interventions and therapy as well as coordination/ case management with clients all within a person centered/ holistic approach.

This would suit a Clinician that is wanting to work within a service that promotes collaboration and puts the participants mental health at the forefront.

About You

* Relevant Qualifications in Social Work, Psychology, or MH Nursing
* Accreditation with a governing body (AASW or AHPRA)
* Knowledge of trauma informed therapeutic frameworks
* Experience supporting adults with moderate to complex mental health challenges
* Down to earth, humble, team player
