About the Role
* Provide personalized support to customers living with disability or older Australians, helping them build independence, identify and achieve goals, and engage in daily life activities.
* Collaborate with customers to develop customized support plans that incorporate their identified wellbeing needs, strengths, and goals.
* Offer transportation, social activities, financial support, appointment attendance, cooking, medication administration, and personal care as required.
Requirements
* Minimum Cert III in individual support, disability, mental health, peer work, or equivalent qualification.
* Valid driver's license and ability to drive large cars and/or vans.
* Police check, WWCC, and NDIS WC (or willingness to obtain before commencing).
* Strong computer skills, with proficiency in Microsoft Office and customer relationship management systems.
