About the Clinical Care Role
The Clinical Care Planner is a vital position in our healthcare organization, where you will work closely with GPs, psychiatrists, and other health professionals to coordinate care for individuals with severe and complex mental illness. Your primary objective will be to develop joint care plans with patients and their support networks to enhance health outcomes, promote effective symptom management, and reduce unnecessary hospitalization.
This role requires a high level of expertise, particularly in using a recovery-oriented framework to deliver clinical care services. You must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
The ideal candidate will have a strong background in mental health nursing, social work, or occupational therapy, with a minimum of 2 years of supervised experience in this field. Additionally, they should be proficient in the use of Microsoft software and databases.
