To provide support to elderly clients in the community to enhance their safety, promote independence and optimise health outcomes in collaboration within a multidisciplinary team. $73,427.00 - $104,753.00 per annum + 12% super & salary packaging | Temporary Full Time until 29 August 2027 | Port Kembla Hospital (PKH) | REQ597273 | Grade: Physiotherapist Level 1/ 2 | Applications close: 22 August 2025 This is a temporary leave relief position until 29 August 2027 with the possibility of extension. ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision-making process around long term care service supports. As the Physiotherapist in ITACS you will provide high quality Physiotherapy services to clients and carers, to ensure the provision of optimal health outcomes for the clients. You will be required to have problem-solving skills to support a client's safe mobility in the home and community setting without direct clinical supervision when completing in-home assessments and interventions. You will also provide care planning, case management and liaise with external stakeholders. The position functions as part of a multi-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care but are unable to return home without additional care and support. As a part of the new Shellharbour Hospital and Integrated Services Project, a new Community Health Centre is being built on the old Port Kembla Hospital site. Located on Dharawal Country, Warrawong Community Health Centre will bring together a range of services including community mental health, drug and alcohol, community nursing, and child and family services. Until the new Community Centre is complete, some allied health services will continue to operate out of the Port Kembla Hospital site. Benefits Of Working With ISLHD.
