Role Overview
Provide comprehensive care across all stages of mental illness, including intake coordination, assessment, and clinical treatment—crisis support and case management—for patients, families, and carers at home, in the community clinic, or in the local Urgent Care Department.
Deliver short‐term crisis intervention and ongoing case management while collaborating with families, carers, and other local health and wellbeing providers to deliver holistic, person‐centred care.
Develop and monitor Individual Service Plans in collaboration with all stakeholders, engage with community services for ongoing support and discharge planning, and participate in education, consultation, and multidisciplinary team activities.
