About the role

The Department of Justice is seeking a dedicated and experienced Chronic Disease Nurse Coordinator to join the health services team within the Western Australian Corrective Services system. This essential role focuses on improving the health and wellbeing of incarcerated individuals living with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions. While the position will be based in Welshpool, the successful applicant will be required to travel to various sites as needed.

Working within a multidisciplinary team, you will provide clinical leadership in the coordination of care, oversight comprehensive assessments, contribute to developing and monitoring individual care plans, and support self-management through education and culturally appropriate health promotion.

As part of your role, you will collaborate closely with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a consistent and patient-centred approach to care. You will also contribute to discharge planning and continuity of care as individuals transition back into the community, including liaising with community health providers and Aboriginal health services where relevant.

You will be responsible for liaising with the local health department to ensure patients are placed on appropriate registers to ensure continuity of care throughout their journey. This position also forms part of the policy review team and will take ownership in informing and developing policies and procedures that relate to Chronic Disease Management.
