Overview
We are seeking an experienced and compassionate Registered Nurse to join our Home Care team as a Clinical Case Manager.
In this role, you will provide clinical oversight, case management and coordination of care to support clients to live safely and independently in their own homes.
This is a rewarding opportunity for an RN who enjoys autonomy, clinical leadership and building strong relationships with clients, families and care teams.
Key Responsibilities
* Manage a portfolio of home care clients, delivering end-to-end clinical case management
* Complete comprehensive assessments, care plans and regular clinical reviews
* Provide clinical oversight and guidance to care staff and support workers
* Ensure care delivery meets Aged Care Quality Standards and Home Care Package requirements
* Monitor clinical risks, incidents and escalation of care as required
* Liaise with GPs, allied health, families and external stakeholders
* Maintain accurate documentation and reporting in line with compliance requirements
About You
* Current AHPRA registration as a Registered Nurse
* Experience in home care, community nursing or aged care
* Strong clinical assessment and care planning skills
* Sound knowledge of Home Care Packages and aged care compliance
* Excellent communication, organisation and time-management skills
* Ability to work autonomously while contributing to a collaborative team
* Current driver's licence and reliable vehicle
