Are you passionate about building meaningful connections and supporting others to succeed? Trilogy Care, one of Australia’s fastest-growing Home Care Package providers, is looking for a Relationship Manager to lead and support our valued network of independent external care coordinators.

This is a unique opportunity for emerging or experienced Aged Care Coordinators who are ready to take the next step in their career - without the demands of direct client care. If you’re feeling the effects of frontline burnout but still want to contribute meaningfully, this behind-the-scenes role offers the perfect balance.

In this role, you’ll play a vital part in strengthening the support framework that enables our coordinators to deliver exceptional care to older Australians. You’ll be making a real impact from a supportive, collaborative space - ideal for those who want to grow, make a difference, and stay connected to their purpose in aged care.

What You'll Do

* Managing a portfolio of established aged care coordinators, keeping them engaged, satisfied, and active
* Be the primary point of contact and trusted advisor for our aged care coordinators beyond their onboarding phase
* Building strong, trust-based relationships through proactive communication and regular check-ins
* Supporting aged care coordinators to deliver exceptional client care by providing guidance, resources, and problem-solving
* Coordinating across internal teams to ensure our aged care coordinators needs are met seamlessly and efficiently
* Monitoring satisfaction, retention, and referral activity to keep a pulse on the success of their coordination business
* Proving established client referrals to our aged care coordinators
* Identifying and addressing risks, inefficiencies, or challenges early
* Providing feedback and insights that help improve the overall relationship management function

What You’ll Drive

* Satisfaction, retention, and referral outcomes
* Transparent, solution-focused communication with our aged care coordinators to support their coordination business
* Fast and effective resolution of partner issues by collaborating across Care, Accounts, Support, and Compliance teams
* Education and troubleshooting to help our aged care coordinators adopt best practices
* Clear reporting on aged care coordinators performance and trends
* A feedback loop that strengthens Trilogy Care’s processes and aged care coordinators experience
* Representation of Trilogy Care’s commitment to service excellence and integrity in every interaction

What You’ll Bring

* Experience as an aged care coordinator, assessor, support worker or ageing related role (preferred)
* Knowledge of the Support at Home Program changes (preferred)
* Understanding the importance of relationship management/support, aged care coordination or stakeholder engagement
* Knowledge of aged care sector, specifically Home Care Package exposure and understanding
* Excellent communication skills with the ability to build rapport, trust, and influence
* Attention to detail and ability to monitor KPIs, risks, and improvement opportunities
* A balance of empathy and commercial awareness to deliver partner success and business outcomes
* Familiarity with CRM systems (Zoho preferred)
