The ACE (Advice, Coordination & Expertise) ED Team is a multidisciplinary team providing care coordination services to clients presenting to the Emergency Department and in the community with clients at risk of presentation. The aim of the role is to reduce presentations / admissions and optimise patient care through the provision of Care Coordination, Rapid Allied Health service and Integrated Care planning. The service operates between 7:30am and 9:00pm, 7 days a week and we are seeking an experienced Therapist to join the team.
- Appropriate tertiary qualification and eligible for discipline specific registration.
- Minimum of 2 years clinical experience in relevant setting including acute and community and working in interdisciplinary teams.
- Experience in assessment of people with complex needs, discharge planning, case conference and care planning within a multi-discplinary team
- A willingness to learn and adapt in a fast paced environment
- Passion for providing patients with the right care at the right time and improving the patient journey
