**Integration & Empowerment Services (IES)** is a local not-for-profit based in Ellenbrook, Western Australia. IES is a registered NDIS service provider delivering a range of holistic support services including premier in-home care, community and social supports, Recovery Coaching, respite assistance, facility-based and community-based social programs and more to residents of the Perth Metro area.
We are currently seeking **_EXPERIENCED SUPPORT WORKERS _**to join our dedicated team.
In this role you will be providing one-on-one support to clients from a range of diverse backgrounds, working together to help meet their goals and needs.

**ABOUT THE ROLE**:
- Work in close communication and cooperation with managing staff regarding all supports provided to participants,
- Provide ongoing support coordination using a person-centered approach to assist participants in capacity and independence building,
- Maintain accurate and up to date records and case notes on the participants,
- Engaging with participants, their supports and 3rd parties, to achieve their NDIS goals,
**QUALITIES**:
- At least 1 year experience working in a similar role
- Maintain a high standard of professional, person-centered client support,
- Organisational and time management skills,
- Ability to work with a high level of autonomy,
- A knowledge and understanding of mental health and mental illness
**REQUIRED QUALIFICATIONS**:
- **Have a Certificate III or IV in Disability, Home and Community Care or Individual Support, or a Certificate Ill in Aged Care**:
- Australian driver's licence with comprehensive car insurance
- First Aid Certificate
- Proof of COVID-19 vaccination
- NDIS Worker Screening Clearance (or willingness to apply)
