Our services covers the West Metropolitan region of Melbourne. Our multi-disciplinary team including allied health, doctors and nurses is committed to providing holistic care to patients and their family ensuring they experience the best quality of life while addressing their physical, emotional, social and spiritual needs.This role is to* Provide a quality, responsive, person- centred social work service to patients, their family and carers using a holistic approach* Complete psycho-social-spiritual assessments and interventions* Be an integral part of multi-disciplinary team caring for palliative care patients in their own home* Join a supportive Allied Health team**What you'll bring**You're resilient, compassionate and have a whole lot of heart.
